A very costly "misunderstanding"

The AAIB final report into the crash of Super Puma helicopter G-REDL in the North Sea off Peterhead has been published here.

One of the conclusions reads..........

36. The use of verbal and email communication between the operator and manufacturer on 25 March 2009 led to a misunderstanding or miscommunication of the issue.


A misunderstanding which resulted in 16 fatalities. :(( :((

Seems to being blamed on "SOMEONE" not carrying out an inspection properly! :-o

Or rather the misidentification of the mag chip debris material combined with a genuine communications foul-up (including the classic assumption error).

Fingers crossed that the safety recommendations prevent a recurrence.

Probably why No.2 was pulled out because of the high amount of debris on the mag chip.

Indeed, but the modern diagnostic systems are much more sophisticated than the very early examples fitted to the Olympus (or at least they are supposed to be). :-?
